Wellness Nurse - Assisted Living Community - LPN or RN - Registered Nurse

D'Youville Life & Wellness Community is seeking a Wellness Nurse to provide direct resident care in our assisted living community. In this role, you will implement and coordinate resident care in collaboration with physicians and nursing staff, manage resident assessments and documentation, and provide functional direction to nursing support staff.

Clinical Setting

D'Youville is a faith-based, nonprofit senior living community serving Greater Lowell and surrounding areas since 1960. We provide short-term rehabilitation, skilled geriatric care, and assisted living services.

Responsibilities

• Implement and coordinate resident care in collaboration with physician and nursing staff
• Provide direct resident care including making rounds, answering call lights, and assisting with medications
• Document all resident care information
• Provide functional direction and assistance to nursing support staff
• Complete new staff orientation as assigned
• Complete monthly and quarterly audits, assessments, and documentation
• Communicate effectively with residents, families, visitors, healthcare team members, and other disciplines
• Participate in screening potential admissions as needed

Required Qualifications

• Massachusetts RN (Registered Nurse) or LPN (Licensed Practical Nurse) license
• Technical or higher education in nursing
• Proficiency with Infection Control and Point-Click-Care

• One year of assisted living facility experience

Schedule

• Part-time and per diem shifts available
• Must be able to work every other weekend

Compensation

• Base pay range: $33.00 to $39.60 per hour based on license type and experience
• Shift differential added to evening shifts
