Weekend Shift Licensed Practical Nurse/ Registered Nurse

Jeffersontown Rehab is seeking Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to provide high-quality, resident-centered care on weekend night shifts. You will work alongside a supportive clinical team in a collaborative, fast-paced environment, delivering direct patient care and monitoring resident conditions to support their overall well-being.

Clinical Setting

This is a long-term care rehabilitation facility serving residents who require ongoing medical support and skilled nursing care.

Responsibilities

• Provide direct bedside care for a wide variety of medical patients
• Prepare and administer medications orally, subcutaneously, and intravenously; record administration and report adverse reactions
• Record patients' medical information and vital signs
• Monitor and adjust specialized equipment used on patients; interpret and record electronic displays
• Change dressings, insert catheters, and start IVs
• Initiate corrective action when patients display adverse symptoms
• Prepare equipment and assist physicians during examinations and treatments
• Respond to life-saving situations based on nursing standards and protocol
• Consult and coordinate with healthcare team members to assess, plan, implement, and evaluate patient care plans
• Participate in discharge planning
• Record all care information concisely, accurately, and completely in a timely manner
• Demonstrate effective verbal and written communication with patients and team members
• Utilize time management skills and contribute valuable ideas and feedback as a team member
• Perform other position-related duties as assigned

• Weekend shifts
• Night shift

Required Qualifications

• Current LPN or RN license in state

Preferred Qualifications

• At least one year of experience as an LPN or RN in a long-term care setting
• Knowledge of Matrixcare
