Assured Home Health, part of LHC Group and the Optum family of businesses, is seeking a Registered Nurse for weekend home health visits in Moses Lake. In this role, you will provide direct nursing care to patients in their homes, develop and implement individualized plans of care in collaboration with physicians and healthcare teams, and supervise care provided by home health aides and licensed practical/vocational nurses.
Responsibilities
- Initiate, develop, implement, and revise plans of care in collaboration with physicians and other healthcare professionals
- Supervise care provided by home health aides and licensed practical/vocational nurses, provide instruction, and assign tasks according to state and federal regulations
- Provide required supervisory visits
- Deliver high-quality clinical services within the scope of practice and infection control standards
- Complete comprehensive assessments including OASIS documentation and medication reconciliation accurately and timely
- Document patient visits per policy and payer requirements
- Make initial and comprehensive nursing evaluation visits to ensure patients meet home health eligibility and medical necessity guidelines
- Determine primary focus of care and develop plans of care within state guidelines with the physician
- Communicate relevant information timely and effectively with agency staff, physicians, patients, and family members regarding patient care issues, visit assignments, schedule changes, orders, and coordination with other clinicians
- Participate in on-call and weekend rotation as needed to meet patient needs
- Assist in orientation of new agency personnel and serve as a preceptor to other staff and students
- Participate in survey readiness activities and performance improvement plans
- Work to reduce unnecessary patient hospitalizations and improve patient safety
- Adhere to and participate in the agency's utilization management model
Required Qualifications
- Current and unrestricted RN licensure in the state of Washington
- One or more years of home health experience
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
- Current CPR certification
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
Preferred Qualifications
- Ability to work independently
- Solid communication, writing, and organizational skills
Schedule
Weekend rotation required. On-call participation as needed to meet patient needs.
Compensation and Benefits
Hourly pay ranges from $45.00 to $60.00 per hour based on full-time employment. Pay is based on local labor markets, education, work experience, and certifications.
