Weekend Nurse Practitioner (Skilled Nursing) Aviata at Engelwood

This position offers an opportunity to serve as the leading internal medicine provider at a skilled nursing facility on weekends (Saturday and Sunday). You will work with both short-term and long-term residents, collaborating with the facility's interdisciplinary team, nursing staff, medical directors, and attending physicians to reduce hospitalizations and provide comprehensive patient care.

Responsibilities

• Evaluate new admissions and coordinate discharge visits
• Assess patients exhibiting changes in condition
• Provide comprehensive health assessments, develop differential diagnoses, and prescribe treatments for acute and chronic illness and disease
• Perform comprehensive physical examinations
• Prescribe all necessary medications and treatments
• Develop and implement patient management policies and procedures
• Provide advanced care planning
• Facilitate referrals to other healthcare professionals and medical facilities
• Communicate with attending physicians and specialists regarding patient care

Required Qualifications

• Advanced Practice Licensure (APN) with an active Florida license
• Active DEA number
• Master's degree
• Previous experience in nursing or other medical fields
• Ability to build rapport with patients and their loved ones
• Ability to thrive in a fast-paced environment
• Excellent written and verbal communication skills
• Strong leadership experience with emphasis on teamwork in a healthcare setting
