Weekend Home Health LPN - Licensed Practical Nurse

Beacon Home Health is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes throughout the Cleveland, Tennessee area and surrounding communities. This is a PRN weekend position that offers the opportunity to build meaningful relationships with patients and families while delivering quality home-based care.

Responsibilities

• Provide skilled nursing care within the scope of practice as defined by Tennessee state nursing laws
• Follow the established plan of care in coordination with the healthcare team
• Instruct patients and families in appropriate patient care and maintenance of a healthy home environment
• Observe, record, and report patient responses to treatment and changes in condition to the supervising nurse and/or physician on a daily basis
• Collaborate with physicians and RNs on patient care
• Document care accurately and promptly
• Maintain the plan of care without alteration

Schedule

• PRN (as-needed) weekend shifts

Clinical Setting

• Patient homes throughout the Cleveland area and surrounding communities
• On-the-road position requiring travel to multiple patient residences daily

Required Qualifications

• One year of clinical LPN experience
• Current LPN license in Tennessee
• Current CPR certification
• Valid driver's license
• Vehicle insurance
• Access to a dependable vehicle or reliable public transportation
• Ability to reliably commute to multiple patient residences daily within the assigned geographic area
