Virtual Neurology Care Nurse Practitioner (Remote) NH

Local Infusion is seeking a Nurse Practitioner licensed in New Hampshire to join our virtual care team. This is a remote, patient-facing clinical role delivering care to patients with neurological conditions, memory disorders, and dementia via telehealth. You will work collaboratively with supervising physicians and the broader care team to increase patient access and reduce time to treatment. You must live in and be licensed in New Hampshire and will work remotely from NH.

Responsibilities

• Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians
• Partner with neurologists and the broader care team to ensure continuity of care
• Educate patients and families on diagnoses, treatment plans, and disease progression
• Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements
• Manage a full patient panel independently with physician collaboration

Required Qualifications

• Active, unrestricted NP license in New Hampshire
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
• Strong clinical judgment and clinical documentation skills
• Excellent communication skills with patients facing cognitive decline and their caregivers

Preferred Qualifications

• Certification in geriatrics, adult-gerontology, or family practice
