Virtual Family Nurse Practitioner (VA License Required) | 3-Week Paid Sabbatical in Q1

Sprinter Health is seeking a part-time Family Nurse Practitioner to conduct virtual wellness visits with patients in their homes. You will leverage your clinical expertise to provide a wide range of healthcare services, including virtual adult and pediatric wellness visits, health assessments, medication reviews, cognitive screenings, and care coordination. This role offers the opportunity to work autonomously while maintaining high-quality standards and delivering personalized, compassionate care to a diverse patient population.

Clinical Setting

You will work virtually, partnering with Sprinter Health's in-home clinical team members (Sprinters) who visit patients' homes. Using modern technology, you will navigate your daily schedule and collaborate with these team members to evaluate vital signs, arrange blood draws, and address specific patient needs.

Responsibilities

• Review your caseload and prepare technology at the start of each day to ensure you have the necessary tools to service patients
• Conduct thorough health risk assessments, medication reviews, and cognitive screenings
• Perform virtual wellness exams and engage in proactive care with patients
• Empower patients with educational information regarding their health and well-being
• Work directly with in-home clinical team members to evaluate vital signs, arrange blood draws, and carry out necessary tasks
• Provide comprehensive care coordination and management, including preventive care interventions, medication management, and specialist referrals
• Coordinate with community resources as needed
• Document findings and maintain accurate records

Required Qualifications

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner license in Virginia
• Prior experience performing wellness visits
• Ability to work independently while maintaining high-quality standards

• Commitment to delivering exceptional customer service
• Technologically savvy and comfortable using laptops, mobile devices for charting, and HIPAA-secure messaging apps for care coordination
• Strong written and verbal communication skills
• Ability to work in a dynamic environment that sometimes requires last-minute problem solving and creative thinking
• Pass national background check and valid clinical license search
