Sprinter Health is seeking a part-time Virtual Family Nurse Practitioner to conduct virtual wellness visits with patients in their homes. You will leverage your clinical expertise to provide a wide range of healthcare services, including adult and pediatric wellness visits, health assessments, medication reviews, cognitive screenings, and care coordination.
Clinical Setting
You will work in a virtual care environment, partnering with Sprinter Health's in-home clinical team members (called Sprinters) who visit patients' homes. You will review patient cases, prepare your technology, and conduct virtual consultations while coordinating with the field team to evaluate vital signs, arrange blood draws, and address specific patient needs.
Responsibilities
- Review your daily caseload and prepare technology to ensure all tools are available for patient care
- Conduct thorough health risk assessments, medication reviews, and cognitive screenings
- Perform virtual adult and pediatric wellness visits and health assessments
- Create warm, welcoming connections with diverse patients and collect relevant health information
- Work collaboratively with in-home Sprinters to evaluate vital signs, arrange blood draws, and complete necessary clinical tasks
- Provide comprehensive care coordination and management, including preventive care interventions, medication management, and specialist referrals
- Empower patients with educational information regarding their health and well-being
- Document findings and maintain accurate patient records
- Coordinate community resources and referrals as needed
Required Qualifications
- Board Certified as a Family Nurse Practitioner
- Active Family Nurse Practitioner license in Texas
- Prior experience performing wellness visits
- Technologically savvy and comfortable using laptops, mobile devices for charting, and HIPAA-secure messaging apps
- Strong written and verbal communication skills
- Ability to work independently and maintain high-quality standards
- Ability to work in a dynamic environment requiring problem-solving and flexible thinking
- Pass national background check and valid clinical license search
