Virtual Clinician Home Evaluations Nurse Practitioner or Physician Assistant (Full Time) - Nebraska

Signify Health, part of CVS Health, is hiring full-time board-certified Nurse Practitioners or Physician Assistants to conduct video visits with Medicare Advantage, Medicaid, and commercial plan members. In this role, you will provide telehealth services exclusively, conducting approximately 30-minute video visits that include medication and medical history reviews, member education, and care coordination. You will work from a private space with adequate internet connectivity to help close gaps in care and connect members with appropriate resources.

Responsibilities

• Conduct online video visits with health plan members, maintaining a private space with adequate internet connection and neutral background
• Complete approximately 30-minute visits including medication and medical history review
• Educate members about their health and answer their questions
• Maintain a strong visit completion rate, consistently completing 220+ visits per month
• Provide 40 hours of availability weekly, consistently providing 320+ capacity units per month
• Participate in regular manager, team, and clinical meetings
• Maintain training, affiliation, and compliance with video visit requirements
• Meet operational, clinical, and member experience key performance indicators and metrics

Required Qualifications

• Board-certified Nurse Practitioner or Physician Assistant
• Active state license in Nebraska (FNP or AGNP certification preferred for NPs)
• Ability to obtain and maintain multi-state licenses in coverage areas as required
• Broadband connectivity with video and audio enabled
• Quiet setting with neutral background for conducting visits
• Ability to uphold health plan member privacy per CVS Health guidelines
• Effective verbal, written, and electronic communication skills
• Ability to work without direct supervision and practice autonomously
• Proficiency with information management and technology
• Specific vision abilities including ability to view and read computer screens, iPads, and other electronic devices

Preferred Qualifications

• Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or similar format

Schedule

• Full-time position
• 40 hours per week

Compensation and Benefits

• Base salary plus incentive structure based on number of visits completed
• Typical pay range: $78,331.00 - $168,714.00 annually
