The LPN Unit Care Coordinator supervises, implements, coordinates, and manages patient care in a post-acute care facility. You will provide clinical leadership on the unit, directing daily nursing and CNA functions while maintaining direct patient care responsibilities. This role requires independent decision-making, strong clinical knowledge, and the ability to respond effectively to emergency situations affecting patient and staff safety.
Responsibilities
- Supervise and direct the daily functions of unit nurses and CNAs to provide floor leadership
- Provide, manage, and coordinate patient care and services through interpersonal contact to help patients attain or maintain the highest practicable physical, mental, and psychosocial well-being
- Accurately prepare and administer medications as ordered by a physician
- Chart appropriately, accurately, and in a timely manner
- Coordinate patient care plans and services with the RN
- Respond in a leadership capacity to emergency situations related to patient and staff safety
- Assist in the evacuation of patients
- Exhibit excellent customer service and maintain a positive attitude toward patients
- Communicate and function productively on an interdisciplinary team
- Maintain professional working relationships with all associates and vendors
- Maintain confidentiality of all proprietary and confidential information
- Understand and follow company policies including harassment and compliance procedures
Required Qualifications
- Completion of an accredited LPN program
- Currently licensed and registered as an LPN/LVN in your applicable state; must maintain an active license in good standing throughout employment
- Advanced knowledge in your field of practice
- Knowledgeable of practices, procedures, laws, regulations, and guidelines governing functions in post-acute care facilities
- Ability to make independent decisions when circumstances warrant such action
- Proficiency in all competency areas including patient rights, safety, and sanitation
- Ability to implement and interpret programs, goals, objectives, policies, and procedures of the department
- Ability to sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours
- Ability to read, write, speak, and understand the English language
- Dependable, regular attendance
- Ability to concentrate and use reasoning skills and good judgment
- Display integrity and professionalism by adhering to the organization's Code of Conduct
Licenses and Certifications
- Active Licensed Practical/Vocational Nurse (LPN/LVN) license in good standing
- CPR certification upon hire or obtain during orientation; must remain current during employment
Preferred Qualifications
- One year of geriatric nursing experience
