This is a 13-week travel assignment for a Registered Nurse in Case Management, based in Saint Helena, California, starting September 14, 2026. You will provide skilled nursing care and case management services in patients' homes through a home health and hospice setting, managing patient caseloads and coordinating care across the interdisciplinary team.

Responsibilities

  • Plan, organize, and direct home care services to meet patients' needs in their homes and manage patient caseloads effectively
  • Perform initial and ongoing nursing assessments and document patient status, response to treatment, and plan of care
  • Provide direct and indirect skilled nursing care within the plan of care; counsel and educate patients and families on nursing and related needs
  • Update nursing plans, medication profiles, and documentation promptly and ensure timely submission of all records per policy
  • Coordinate patient care with other disciplines and incorporate input from the care team to adjust the plan of care as goals are clarified
  • Communicate changes to goals or status with patients and families and serve as a resource to patients, families, and other care team members

Schedule

  • 5 days per week, 8-hour shifts (day shift)
  • Guaranteed 36 hours per week
  • 13-week contract with potential extension opportunities

Compensation and Benefits

  • Weekly pay: $2,292 to $2,357 per week

Required Qualifications

  • Active Registered Nurse (RN) license in California
  • CPR or Basic Life Support (BLS) certification
  • At least two years of recent nursing experience
  • EPIC experience required
  • Valid driver's license and minimum age of 21 years
  • Current automobile insurance and a reliable personal automobile
  • Ability to work in a home health environment

Preferred Qualifications

  • Bachelor's Degree in Nursing (BSN)
