Join our telehealth team as a Family Nurse Practitioner providing virtual care to patients across multiple settings through the NeverAlone program. This full-time, remote position focuses on serving underserved populations, particularly in skilled nursing facilities and long-term care settings. You will conduct virtual patient visits, perform thorough assessments, develop care plans, and maintain accurate documentation while collaborating with interdisciplinary care teams.
Clinical Setting

- Experience in acute, post-acute, nursing home, palliative care, and/or emergency department settings strongly preferred
Responsibilities
- Provide telehealth care to patients of multiple settings through the NeverAlone program
- Serve patients of all ages in diverse settings with strong emphasis on underserved populations in SNF/LTC
- Perform thorough assessments and determine the best plan of care
- Ensure accurate, timely, and complete documentation of all encounters
- Collaborate with care teams to support continuity of care
- Deliver compassionate, patient-centered service to underserved communities
Schedule
- Full-time position: 40 hours per week
- Primarily focused on daytime rounding with at least three daytime rounding shifts weekly
- Remaining hours may include evenings, weekends, and flexible coverage shifts as needed
- Remote work, location anywhere in the U.S.
Required Qualifications
- Active Nurse Practitioner license in at least two of the following states: Maryland, West Virginia, Virginia, Ohio, or Indiana
- DEA license required
- 3-5+ years of experience as a Nurse Practitioner
- Family NP certification
- Strong ability to interpret patient symptoms and apply best practices in treatment
- Excellent communication and documentation skills
Preferred Qualifications
- Experience in acute, post-acute, nursing home, palliative care, and/or emergency department settings
