Job Description

This position supports the clinical department and program office in a hospice setting, providing essential clerical and administrative coordination for medical record management, chart audits, durable medical equipment (DME) management, and aide scheduling.

As a Team Coordinator LPN, you will work closely with the Clinical Director and interdisciplinary team to ensure accurate and timely documentation, proper patient record maintenance, and smooth operational support for the hospice program.

Responsibilities

  • Maintain and update patient admission files in the patient accounting system (PAS)
  • Complete admission audits and communicate issues or problems to the Clinical Director
  • Input updates into the software system regarding insurance, level of care, staff assignments, and discharge information
  • Scan and file all paper clinical data, hand-signed orders, and consents into patient records
  • Ensure plan of care and comprehensive assessment documents are opened in a timely manner to allow staff to complete documentation before team meetings
  • Resolve plan of care and comprehensive assessment documents once approved by the Clinical Director or designee
  • Track and email certification and recertification statements, plans of care, comprehensive assessments, and orders as appropriate
  • Run weekly unresolved orders reports to track unsigned orders
  • Maintain the patient on-call book
  • Contact physician offices, hospitals, or nursing facilities to obtain current information, signatures, and other documentation pertinent to patient admissions and hospice charts
  • Assist with providing information for annual reports and other requested patient data
  • Receive visit logs from all staff and input logs from staff not using electronic documentation into the software system
  • Compare visit logs to contact visit reports weekly
  • Track and order durable medical equipment and medical supplies
  • Participate in team meetings as requested and ensure all documentation and electronic records are present as needed
  • Maintain team meeting sign-in sheets and ensure team agendas are attached
  • Close clinical and bereavement charts and prepare charts beyond thirteen months for off-site storage
  • Communicate with the Clinical Director regarding chart storage arrangements
  • Maintain and order office supplies
  • Schedule and coordinate hospice aide daily assignments to ensure patients receive needed services
  • Perform intake tasks as assigned
  • Register staff and patients with vendors such as pharmacy, EMR, and supply companies as directed
  • Assist with human resource functions as directed
  • Distribute satisfaction surveys and compile results
  • Type letters, reports, and other materials as requested
  • Answer the telephone professionally and channel calls appropriately
  • Interact with other disciplines as needed to ensure support
  • Participate in Quality Assessment and Performance Improvement activities
  • Complete other duties as assigned or requested

Required Qualifications

  • High school diploma or equivalent
  • One year of medical records experience, preferably in home health or similar operation
  • Good computer skills
  • Positive interpersonal skills and good organizational skills
  • Ability to function as a member of an integrated healthcare work team
  • Flexible, cooperative, and self-directed approach to fulfilling roles

Preferred Qualifications

  • Electronic medical record (EMR) experience
