Holon Health is seeking a Nurse Practitioner licensed in Missouri to provide telehealth-based care to justice-involved individuals living with Substance Use Disorder (SUD). You will work collaboratively with Care Navigators, Recovery Coaches, Case Managers, judicial partners, and community-based organizations to deliver integrated, whole-person care that addresses clients' mental, emotional, and physical health needs.

This position offers flexibility and can be structured as either full-time (40 hours per week) or part-time (20 hours per week) depending on your availability and interest.

Responsibilities

  • Conduct health evaluations to assess substance use, co-occurring conditions, and overall functioning
  • Diagnose health conditions and create individualized treatment plans
  • Prescribe and manage medications, monitor for side effects, and adjust treatment as needed
  • Collaborate with the Holon care team and external providers to deliver coordinated, whole-person care
  • Serve as a clinical consultant on complex or high-need cases
  • Educate clients and families on treatment plans, medications, and recovery support strategies
  • Maintain accurate and timely documentation in compliance with ethical, legal, and medical standards
  • Participate in team meetings, professional development, and continuous training

Required Qualifications

  • Active and unrestricted Nurse Practitioner license in Missouri
  • Active DEA license (and CDS where applicable)
  • Several years of experience in behavioral health, addiction medicine, or psychiatry
  • Strong documentation skills and proficiency in EHR platforms
  • Clear and confident virtual communication skills with both clients and team members
