The Strong Ties Clinic seeks a Registered Nurse II for our outpatient adult mental health clinic. You will provide direct patient care to a culturally diverse population as part of a multidisciplinary team, working primarily day shifts in a community-based psychiatric setting.
Responsibilities
- Conduct telephone triaging and outreach phone calls to engage patients
- Manage Long Acting Injectable clinic visits
- Obtain medication use history
- Process medication refills and assess medication side effects
- Participate in and support providers with medication clinic visits
- Conduct primary care screening
- Provide patient and family teaching
- Collaborate with community providers and systems to facilitate treatment planning
- Support peers in the delivery of patient care services
- Assume expanded roles including charge nurse, preceptor, and committee member responsibilities as needed

Strong Ties Clinic is located at 2613 W Henrietta Rd, Brighton, New York. The clinic serves adults with mental health needs in an outpatient environment.
Required Qualifications
- Active New York RN license
- Experience in psychiatric or mental health nursing
- Strong communication skills
Schedule
- Full-time position, 40 hours per week
- Primarily day shift (UR - Day)
- Regular employment
Compensation
- Hourly rate: $38.76 - $52.33
- Individual salaries will be determined based on market data, education, experience, qualifications, expertise, and internal equity considerations
