State Traveler Provider (Nurse Practitioner / Physician Assistant)

Advantmed is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join our full-time provider network as a State Traveler Provider. In this role, you will conduct comprehensive in-home health assessments and Annual Wellness Visits for Medicare and other eligible patient populations throughout Southern Michigan. Your work will support accurate clinical documentation, identification of care gaps, and delivery of high-quality, value-based healthcare.

Clinical Responsibilities

• Travel to members' homes and conduct comprehensive health evaluations
• Perform Annual Wellness Visits and health assessments for members with chronic conditions
• Complete comprehensive medical history reviews, physical examinations, review of systems, and vital signs collection
• Perform medication review and reconciliation
• Conduct preventive health screenings including Hemoglobin A1C, spirometry, kidney health evaluation, diabetic retinal exam, fecal immunochemical test, and bone mineral density screening
• Deliver preventive health education and counseling on disease prevention, medication adherence, and chronic condition management
• Conduct goals-of-care discussions when appropriate
• Identify clinically relevant conditions and diagnoses for risk adjustment and care management
• Document findings accurately and communicate information for post-visit summaries
• Assist in identifying and closing quality care gaps, including preventive screenings, laboratory testing, and follow-up care
• Collaborate with interdisciplinary teams to improve clinical quality measures and patient outcomes
• Adhere to HIPAA regulations and maintain confidentiality of Protected Health Information
• Complete accurate, compliant, and timely EMR documentation
• Follow organizational policies, clinical protocols, and regulatory requirements
• Maintain professional licensure and certifications

Travel Requirements

This position supports members throughout Southern Michigan with approximately 75% travel required. Overnight travel may be necessary based on assignment locations and member scheduling. Hotel accommodations, meals, and mileage reimbursement are provided for eligible business travel. Travel schedules will be coordinated in advance whenever possible.

Required Qualifications

• Active and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in Michigan
• National board certification applicable to NP or PA specialty
• Minimum of 3 years of direct patient care experience
• Experience in primary care, family practice, internal medicine, geriatrics, urgent care, or related settings
• Valid driver's license and reliable transportation
• Ability and willingness to travel extensively throughout Southern Michigan

Physical Requirements

• Ability to travel frequently by automobile throughout assigned territories
• Ability to enter and work within patient homes
• Ability to lift and carry clinical equipment as needed
• Ability to sit, stand, bend, and walk for extended periods during patient visits

Preferred Qualifications

• Experience with Medicare populations
• Prior experience conducting in-home assessments or risk adjustment visits
• EMR proficiency
• Bilingual language skills
• Familiarity with value-based care and quality gap closure initiatives
