SOC Registered Nurse-Home Health

BAYADA Home Health Care is hiring a Registered Nurse to perform start of care home health visits throughout Monroe County. This role offers flexible schedules, one-on-one patient care, and strong work/life balance.

Clinical Setting

You will provide nursing care in patients' homes throughout designated geographic territories in Monroe County, performing initial assessments and establishing care plans based on individual patient needs.

Responsibilities

• Follow designated care plans in accordance with patient needs
• Make home visits to clients in assigned geographic territories
• Administer medications, perform wound care, and carry out treatments and procedures as assigned
• Monitor clients' conditions and report changes to the Clinical or Client Services Manager
• Execute and properly document physicians' orders
• Perform client assessments as necessary
• Provide case management and care coordination
• Accurately document observations, interventions, and evaluations using electronic medical records on a tablet device

Required Qualifications

• Current active Registered Nurse license in Pennsylvania
• Minimum one year of recent, verifiable clinical (medical/surgical) experience
• Graduation from an accredited and approved nursing program (school transcript or diploma required)
• Ability to work independently and manage time effectively
• Strong interpersonal skills
• Solid computer skills
• Ability to travel to cases as assigned

Preferred Qualifications

• Prior home care experience
• Prior experience with electronic medical records (EMR)

Compensation

Base pay ranges from $52.00 to $64.00 per hour, depending on qualifications and subject to negotiation.
