Senior Nurse Practitioner (Skilled Nursing) Avir at Colonel

Avir Health Group is seeking a qualified Nurse Practitioner to join our team in a Skilled Nursing Facility (SNF) setting. In this clinical leadership role, you will work collaboratively with the facility's care team to provide comprehensive care for both short- and long-term residents.

Responsibilities

• Oversee admissions and discharges
• Conduct monthly stability checks and evaluations for changes in resident condition
• Develop and implement advanced care planning
• Perform comprehensive assessments
• Prescribe treatments and manage clinical care
• Serve as a clinical leader within the interdisciplinary care team

Required Qualifications

• Active Texas Advanced Practice Licensure (APN)
• Active DEA License
• Master's degree in Nursing
• Previous nursing or clinical experience
• Strong communication and teamwork skills
