School Nurse

Homecare with Heart is seeking a skilled nurse to provide home health services on a part-time basis. This position is Fridays only from 6 a.m. to 4 p.m. You will work under the direction and supervision of the RN, performing patient care in accordance with physician orders and the established plan of care.

Responsibilities

• Administer medication in accordance with standards of practice
• Perform patient treatments according to the plan of care
• Assist patients in learning appropriate self-care techniques and encourage independence
• Monitor patient condition, document changes, and report to the Clinical Director
• Prepare clinical and progress notes to document care provided
• Observe aseptic technique and practice Standard Precautions at all times
• Promote activities to achieve a safe and healthy environment
• Work collaboratively with the clinical team to develop and improve the plan of care
• Assist patients in achieving established goals in their plan of care
• Participate in patient care meetings, in-services, orientation, and staff meetings
• Maintain productivity according to agency standards
• Adhere to all agency policies, procedures, and HIPAA guidelines
• Request supervisory assistance when appropriate
• Perform other duties as assigned

Required Qualifications

• Active RN or LPN license in the State of Ohio, in good standing
• Current BLS CPR certification
• Ability to read and understand clinical documents including plans of care, flow charts, patient charts, and agency policies
• Knowledge of medical terminology
• Ability to use a computer, laptop, or tablet
• Math skills including ability to add, subtract, multiply, and divide whole numbers
• Ability to think independently, recognize potential problems, gather information, and know when to contact a supervisor
• Commitment to patient confidentiality and privacy protection
• Ability to provide exceptional patient care while adhering to agency and program rules
• Type and computer skills

Preferred Qualifications

• One year or more experience as an RN or LPN in home health
