School-Based Health Clinical Manager [Nurse Practitioner or Physician Assistant]

Third Street Family Health Services seeks an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to serve as School-Based Health Clinical Manager in Mansfield, Ohio. This full-time, onsite position combines direct patient care with clinical and operational leadership of an established school-based health program serving children, adolescents, and families.

This role is designed for an experienced provider comfortable practicing independently as the primary clinical provider at the site while supporting a multidisciplinary team. You will balance patient care responsibilities with program leadership, staff supervision, and coordination with school and community partners.

Clinical Setting

You will work in a school-based health environment within a regional not-for-profit community health center. Third Street Family Health Services operates across eleven locations in Richland, Ashland, and Crawford counties, providing medical, dental, OB/GYN, pediatric, community outreach, and behavioral health services.

Responsibilities

Patient Care

• Provide primary care to students and other patients within your licensure, credentialing, privileges, and scope of practice
• Deliver age-appropriate, student-centered care focused on children, adolescents, and families
• Promote accessible, coordinated, equitable, and high-quality healthcare
• Maintain timely clinical documentation, encounter closure, inbox management, and other provider responsibilities
• Participate in quality improvement, peer review, audits, and other clinical quality activities
• Serve as a clinical resource and mentor to school-based health team members

Program Leadership

• Provide day-to-day leadership and oversight of the established school-based health program
• Directly supervise assigned staff and provide coaching, feedback, performance management, and professional development
• Coordinate multidisciplinary team activities and facilitate effective team communication
• Establish expectations for teamwork, communication, service delivery, patient experience, and accountability
• Identify operational barriers and partner with organizational leadership to resolve them
• Support hiring, onboarding, training, orientation, and competency development of assigned staff

Quality and Operations

• Ensure consistent workflows for scheduling, patient flow, documentation, care coordination, quality, safety, and service delivery
• Monitor clinical and operational performance and use available data to identify trends and opportunities for improvement
• Partner with administrative and clinical leaders regarding staffing, facilities, technology, supplies, scheduling, and other operational needs
• Ensure school-based health operations align with Third Street policies and applicable HIPAA, FERPA, HRSA, clinical, regulatory, and school-based health requirements

School and Community Partnerships

• Serve as a clinical and operational liaison between Third Street and participating schools
• Build positive working relationships with school administrators, teachers and staff, students, families, and community partners
• Help identify and address barriers to student access, engagement, and care coordination
• Participate in school and community outreach, health education, wellness activities, and other program activities as appropriate

Required Qualifications

• Current Ohio license as a Nurse Practitioner or Physician Assistant
• Board certification appropriate to professional discipline
• Current certifications, credentials, and clinical privileges required for the position
• Valid driver's license
• Experience providing primary care or other relevant clinical services
• Experience working in a multidisciplinary healthcare environment
• Strong clinical judgment and communication skills
• Ability to work independently and balance direct patient care with administrative and leadership responsibilities
• Demonstrated ability to supervise, coach, and develop staff
• Knowledge of primary care, preventive health, integrated healthcare delivery, and care coordination
• Knowledge of applicable healthcare privacy, regulatory, and compliance requirements, including HIPAA and FERPA
• For Physician Assistant applicants: Completion of required clinical training/hours and eligibility to practice without an ongoing onsite physician training requirement

Preferred Qualifications

• Experience providing care to children and adolescents
• Experience working with families and school-age populations
• Previous school-based health experience
• Supervisory or healthcare leadership experience
• Experience in a Federally Qualified Health Center (FQHC) or community health center
• Experience with quality improvement, population health, or care coordination
• Experience working with multidisciplinary teams and community partners
• Master's degree or other advanced education appropriate to your professional discipline
