RN, Registered Nurse Hospice

VitalCaring is seeking a Registered Nurse (RN) to deliver compassionate, comfort-focused hospice care in a field-based setting. In this role, you will serve as the clinical leader and advocate for patients and families during end-of-life care, managing individualized care plans centered on pain and symptom management, quality of life, and patient goals.

You will work autonomously while collaborating with an interdisciplinary team including physicians, social workers, chaplains, and aides. This position offers high clinical autonomy with strong team support in a meaningful, one-on-one care environment.

Responsibilities

• Conduct comprehensive patient and family assessments
• Develop and manage individualized, comfort-focused care plans
• Deliver skilled nursing care with focus on pain and symptom management
• Act as case manager, coordinating care across the interdisciplinary team
• Monitor patient condition and adjust care plans to maintain comfort
• Provide education, emotional support, and guidance to patients and families
• Help families navigate complex decisions with empathy and clarity
• Maintain accurate, timely documentation
• Participate in interdisciplinary team meetings and care planning
• Support quality and compliance standards

Clinical Setting

• Field-based care in homes, facilities, and community settings

Required Qualifications

• Active RN license or compact eligibility
• Graduate of an accredited nursing program
• Minimum 1 year of clinical nursing experience
• Valid driver's license and reliable transportation
• Current CPR certification
• Basic EMR proficiency
• Strong clinical judgment and ability to manage a caseload independently
• Excellent communication skills in sensitive situations

Preferred Qualifications

• Experience in hospice, palliative care, or home health
• Strong pain and symptom management skills
• Case management and interdisciplinary care experience
• Ability to quickly build trust with patients and families
• Experience with Homecare Homebase (HCHB)
