RN Registered Nurse Home Health UAS Assessments | Hybrid

This long-term hybrid position offers an experienced Registered Nurse the opportunity to conduct comprehensive in-home assessments for patients in the Bay Ridge, Crown Heights, Midwood, Flatbush, Williamsburg, and South Brooklyn areas. As a Home Health Nurse Assessor, you will evaluate patients to determine the care and services they need, working independently within a community-based care model that emphasizes geriatric health and patient-centered assessment.

This is a local position only. Hybrid work arrangement with flexible scheduling allows you to start earlier in the day and complete up to 3 assessments per day. Weekend work is available.

Responsibilities

• Conduct in-home patient assessments including OASIS evaluations
• Evaluate patients to determine appropriate care and services
• Provide comprehensive assessments for homebound patients
• Work independently in the field across assigned service areas

Required Qualifications

• Active RN license
• Minimum 2 years of home-based healthcare experience within the last 10 years, including at least one of the following:
• Field work with a Certified Home Health Agency (CHHA)
• Work with a Licensed Home Care Services Agency (LHCSA)
• Experience with Managed Long-Term Care (MLTC) programs
• Conducting home-based patient assessments such as OASIS
• Providing primary care to homebound patients
• Care management for individuals receiving home and community-based services, hospice, or assisted living
• Home care experience as an LPN or Home Health Aide caring for at least 2 patients per week
• Local candidate status required

Compensation

• Rate increase available for caregivers submitted during May with start date in June
