RN Registered Nurse Home Health

PruittHealth @ Home is seeking a Registered Nurse to provide skilled nursing care to patients in their homes. This is a part-time position with special consideration for applicants who currently live in or are commutable to Chattooga County, Georgia.

You will deliver competent nursing care and interventions according to state and industry standards, working under physician supervision. You will evaluate and initiate patient care, conduct clinical outcomes monitoring, and manage patient care plans in collaboration with the interdisciplinary team. Your responsibilities include supervising care provided by Home Health Aides and Licensed Practical Nurses, conducting field visits, patient and family education, and completing comprehensive documentation including OASIS assessments, admissions, recertifications, and subsequent visit notes.

You will assess, monitor, and report on patient health and condition progress, ensuring patients' rights are honored and equitable treatment is provided. You will maintain continuity of care by making referrals to other disciplines as needed and adhering to physician-approved care plans. Additional responsibilities include meeting agency productivity standards, participating in surveys and mandatory in-services, following established safety regulations, and maintaining current licensure and certifications.

Required Qualifications

• Nursing degree from an accredited college or university or graduate of an accredited/approved RN program
• Minimum of one year of RN experience
• Current, active, and unrestricted Registered Nurse (RN) licensure in state of practice
• Current CPR certification
• Valid driver's license
• Reliable transportation
• Liability insurance to complete home visits

Preferred Qualifications

• Bachelor of Science in Nursing (BSN)
• Acute care experience with adult or aging populations
• Home health and/or skilled nursing experience
