RN - Registered Nurse - Home Health

Providence Home Care of Florida, a Medicare-certified home health agency serving Pasco and Pinellas County, is seeking an experienced Registered Nurse to provide skilled nursing care to patients in their homes on a per diem basis. This role focuses on one-on-one patient care delivered through individual visits rather than shifts, allowing for flexibility and work-life balance.

Clinical Setting

You will deliver intermittent skilled nursing care in patients' homes throughout the West Pasco territory. Care is provided in accordance with physician orders and the established plan of care under the direction of the Director of Clinical Services/Clinical Manager.

Responsibilities

• Provide skilled nursing services in accordance with the individualized plan of care
• Conduct initial evaluation visits and regularly reevaluate patients' nursing needs
• Initiate plans of care and make necessary revisions based on patient condition
• Provide nursing services requiring substantial specialized skills
• Initiate appropriate preventive and rehabilitative nursing procedures
• Prepare clinical and progress notes for each patient visit and care conference summaries in a timely manner per agency policy
• Inform personnel of changes in patient condition and needs
• Counsel patients and families/significant others on meeting nursing and related needs
• Participate in and present in-service programs
• Process physician orders and notify physicians of patient needs and condition changes
• Complete certification, recertification orders, and discharge summaries
• Determine the amount and type of nursing care needed for each patient
• Refer patients to Physical Therapy, Speech Language Pathology, Occupational Therapy, and Medical Social Work as appropriate
• Supervise and teach other nursing personnel
• Provide total patient care as needed
• Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition documentation per agency timeframes

Required Qualifications

• Graduate of an approved school of professional nursing
• Current Florida RN license
• Current CPR certification
• Current TB test
• Health Statement/physical clearance to work
• Eligible for Level 2 background check

Preferred Qualifications

• One year of nursing experience
