RN Registered Nurse Home Health

As a Nurse Case Manager with Amedisys, you will guide patients and families through the care continuum in a home health setting. You will assess patient needs, coordinate care, support discharge and transition planning, and manage ongoing outpatient care. Working under the supervision of an RN or MD, you will help ensure patients receive the appropriate level of care while building trusted relationships and supporting them through each stage of their health journey.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care
• Make referrals to other disciplines as indicated by patient needs or document rationale for not doing so
• Promote patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning self-care techniques
• Supervise LPNs and HHAs
• Complete documentation timely and accurately
• Regularly communicate patient progress to the clinical manager and care team
• Plan and provide staff education
• Perform on-call responsibilities and on-call services to patients and families as assigned
• Participate in clinical development and continuing education programs
• Perform other duties as assigned

Required Qualifications

• Current RN license specific to the state(s) you are assigned to work
• Current CPR certification
• One or more years of clinical experience as a Registered Nurse
• Valid driver's license, reliable transportation, and liability insurance

Compensation

Annual total cash target pay ranges from $67,905 to $101,857 based on full-time employment at full productivity. Compensation is structured as follows:

• Per visit point rate: $39.18 to $58.76 per visit point
• Hourly rate: $32.65 to $48.97 per hour

When conducting patient visits, you will be paid per visit point rate compensation. Your per visit pay is calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each visit type is assigned a certain number of productivity points inclusive of direct and indirect patient care activities. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. You will be paid your hourly rate for certain non-visit activities such as orientation. Compensation is based on several factors including local labor markets, education, work experience, and may increase over time based on productivity and performance in the role.
