RN (Registered Nurse) Home Health Case Manager

Phoenix Home Care and Hospice is seeking a compassionate RN Case Manager to manage patient caseloads in home health settings across Kansas City, MO and surrounding areas. In this role, you will provide direct patient care, supervise LPN visits, and manage your own schedule. You will conduct varied weekly visits including routine care, evaluations, and starts of care, ensuring no two days are the same.

This position leverages innovative technology to streamline your workflow. Roger, an AI-powered charting assistant, enables real-time documentation during patient visits, allowing you to complete your workday on time without evening paperwork.

Responsibilities

• Manage your own caseload including supervision of LPN visits
• Conduct varied weekly visits including routine care, evaluations, and starts of care
• Educate patients on health conditions including CHF, COPD, Diabetes, Wound Care, and Medication Management
• Collaborate with physicians to obtain orders and write diagnosis-driven plans of care
• Work alongside a strong therapy team including PT, OT, and Speech

Required Qualifications

• Active Missouri or Compact RN License
• Valid Missouri Driver's License
• Strong clinical skills in physical assessment and diagnostic interpretation
• Organized, self-directed approach to managing your own caseload
• Ability to communicate effectively with patients, families, and physicians

Preferred Qualifications

• Home health experience

Schedule and Employment

• Full Time, Part Time, and PRN availability

Compensation and Benefits

• Pay Range: $80,000 – $95,000
