RN (Registered Nurse) Home Health Case Manager

This position is a full-time RN Case Manager role in home health, based in Wichita, Kansas and serving surrounding areas. You will manage a caseload of home health patients, supervising LPN visits and coordinating care with Physical Therapy, Occupational Therapy, and Speech Therapy teams. You will conduct patient assessments, develop personalized care plans aligned with physician orders, and support patients managing conditions including CHF, COPD, diabetes, wounds, and medication regimens.

Schedule

• Monday through Friday, day schedule
• On-call rotation approximately every 11-14 weeks, consisting of 2 weekday nights per month and 1 holiday rotation per year
• You build your own visit schedule, allowing flexibility for personal appointments and life needs

Compensation and Bonuses

• Base salary: $78,000 to $88,000
• $2,500 sign-on bonus
• Pay-per-visit model: earnings often exceed the stated range based on visit completion
• Mileage reimbursed on every visit
• Out-of-town visit pay for qualifying trips beyond your territory

Required Qualifications

• Active Kansas RN license or Compact RN license
• Valid driver's license
• Strong organizational, clinical, and communication skills
