RN (Registered Nurse) Home Health Case Manager

Phoenix Home Care and Hospice is seeking a compassionate RN Case Manager to manage patient caseloads in home health settings across Kansas City, MO and surrounding areas. In this role, you will provide direct patient care, supervise LPN visits, and manage your own schedule. You will work with patients managing chronic conditions including CHF, COPD, Diabetes, and wound care, while collaborating with physicians and interdisciplinary therapy teams to develop and implement individualized plans of care.

This position offers flexibility with full-time, part-time, and PRN availability. Phoenix has implemented Roger, an AI-powered charting assistant, to streamline documentation during patient visits so that charting happens in real time and your workday actually ends when you leave.

Responsibilities

• Manage your own caseload including supervision of LPN visits
• Conduct varied weekly visits including routine care, evaluations, and starts of care
• Educate patients on health conditions including CHF, COPD, Diabetes, wound care, and medication management
• Collaborate with physicians to obtain orders and write diagnosis-driven plans of care
• Work alongside PT, OT, and Speech therapy teams

Required Qualifications

• Active Missouri or Compact RN License
• Valid Missouri Driver's License
• Strong clinical skills in physical assessment and diagnostic interpretation
• Organized, self-directed approach to managing your own caseload
• Ability to communicate effectively with patients, families, and physicians

Preferred Qualifications

• Home health experience

Schedule and Compensation

• Full-time, part-time, and PRN positions available
• Pay range: $80,000 – $95,000
