RN Registered Nurse Home Health

AdventHealth is seeking a Registered Nurse for Home Health to provide skilled nursing care to patients in their homes across multiple counties in the Tampa Bay and central Florida regions. This full-time position offers the opportunity to work independently while managing a caseload of home care patients, coordinating care with physicians and interdisciplinary teams, and utilizing motivational interviewing and health coaching techniques to engage patients and families in their care.

Responsibilities

• Evaluate home environments for safety, infection control, and community resource needs
• Review patient history, physical diagnostics, and laboratory data; report abnormal results to physicians
• Implement individualized care plans through direct patient care, coordination, delegation, and supervision of healthcare team activities
• Provide skilled nursing care, preventative and rehabilitative procedures, and prescribed treatments in various home settings
• Assess physical, functional, psychosocial, cultural, and cognitive status of patients; identify discharge planning needs
• Formulate patient-specific care plans with patients, families, and physicians, establishing individualized, realistic, and measurable goals
• Inform physicians, clinical managers, and healthcare teams of changes in patient condition and needs
• Maintain updated clinical records, meeting documentation deadlines for certification, re-certification, and care plan updates
• Optimize daily schedules to support productivity, efficiency, and best practice visit utilization
• Perform other duties as assigned

Clinical Setting

You will provide home-based care across one of the following coverage areas: Hillsborough County, Pasco County, Pinellas County, Polk County, or Sebring/Highlands/Hardee.

Schedule and Shift

• Full-time position
• Monday through Friday, 8:00 AM to 5:00 PM
• Weekend on-call rotation required
• Company-provided phone and laptop/iPad
• Mileage reimbursement provided at $0.76 per mile

Required Qualifications

• Associate's degree in Nursing
• Current Registered Nurse (RN) license
• Valid driver's license
• Auto insurance
• Basic Life Support (BLS) CPR certification
• One or more years of relevant clinical nursing experience
• Functions with a high degree of independence
• Ability to delegate tasks to appropriate personnel as indicated by skill level and professional standing
• Strong computer and technology skills

Preferred Qualifications

• Bachelor's degree in Nursing
• Recent, relevant experience in a Medicare-certified home health agency as a case manager
• Working knowledge of community resources and ability to refer patients and families appropriately
• Knowledge of home care regulations and third-party reimbursement as it impacts care delivery
• Current IV therapy skills
• Certified Oasis Specialist-Clinical (COSC) certification

Additional Requirements

• Certain positions are subject to Florida Level 2 background screening, including fingerprinting, as required by state law
