Phoenix Home Care & Hospice is seeking a Registered Nurse (RN) to serve as a Home Health Case Manager in Jefferson City, Missouri and surrounding areas. In this role, you will provide skilled nursing care to patients in their homes while coordinating their overall plan of care. You will manage an assigned home health nursing caseload, conduct skilled nursing visits and evaluations, and supervise LPN visits while coordinating care among healthcare providers.
Responsibilities
- Manage an assigned home health nursing caseload
- Provide skilled nursing visits, evaluations, and starts of care
- Supervise LPN visits and coordinate care among healthcare providers
- Complete comprehensive nursing assessments and develop individualized plans of care
- Communicate with physicians to obtain orders and establish diagnosis-driven plans of care
- Coordinate with insurance companies and other healthcare professionals
- Educate patients and families about conditions including CHF, COPD, diabetes, wound care, and medication management
- Collaborate with Physical Therapy, Occupational Therapy, and Speech Therapy teams
- Provide hands-on patient care in patients' homes
- Use clinical judgment, physical assessment skills, and diagnostic information to support quality patient outcomes
Required Qualifications
- Missouri RN license or Compact RN license
- Valid driver's license
- Reliable vehicle with current auto insurance
- Strong assessment, communication, and clinical decision-making skills
- Ability to work independently while collaborating with an interdisciplinary care team
Preferred Qualifications
- Home health nursing experience
- Critical care experience

Compensation and Benefits
- $5,000 sign-on bonus
- $65,000–$90,000 annual pay range
