Job Description

Home Care Network, Inc. is seeking an experienced RN Case Manager and Recertification Nurse for a PRN position serving the far-western Northwestern Ohio region. This role offers the opportunity to build a long-term employment relationship with a nurse-owned, locally led home health agency specializing in care for Medicaid-eligible patients including infants, pediatrics, adults, and seniors.

You will coordinate and provide patient care while ensuring optimal outcomes through case management, patient assessment, treatment plan documentation, and discharge planning. This position combines recertification visits, starts of care, admissions, transitional care, resumptions of care, clinical reviews, and other assessments. You will also perform weekly medication setups, new hire orientations, in-service training, and supervisory reviews while supporting state survey activities.

Clinical Setting

This is a home-based care position serving clients across Defiance, Paulding, and Van Wert Counties in Ohio, plus surrounding areas within a 1-hour radius. You will work closely with and report directly to the Northwest Ohio Regional Program Administrator and the Director of Clinical and Administrative Operations. The agency's Northwest Ohio Regional Office is located at 134 W South Boundary St., Suite G1-2, Perrysburg, OH 43551.

Responsibilities

  • Perform client assessments while directing, planning, and initiating appropriate actions in home care situations
  • Manage and supervise a team of RNs, LPNs, Home Health Aides, and Personal Care Aides
  • Document, communicate, and coordinate patient status and changes in condition to the care team
  • Perform case management, starts of care, admissions, recertifications, transitional care, and resumptions of care
  • Conduct clinical reviews and other assessments and services
  • Perform weekly medication setups
  • Conduct new hire orientations and in-service training
  • Provide supervisory reviews and input to influence training, education, care practices, and outcomes
  • Perform job duties while promoting personal safety and a safe environment for clients and coworkers
  • Communicate with physicians, agency staff, service administrators, other caregivers and clinicians, and clients and their families
  • Provide occasional in-office state survey support as needed (1-2 times per year)

Schedule

This PRN position consists of 1-2 consistent weekly medication setups and 1-10 recertifications per week, Monday through Friday between 8:00 AM and 5:00 PM. Occasional early-evening and rare weekend visits may occur but are not required. No overnight or overtime shifts are required. The flexible schedule allows many nurses to pair this opportunity with other employment. The agency prioritizes scheduling visits as close to your home as possible and will work around existing commitments so long as you can accept most or all client care requests and support state surveys.

There is potential for growth into a part-time or full-time RNCM position as the agency grows over the next 6-12 months, though additional hours cannot be guaranteed at this time. Contractors and contracted arrangements will also be considered.

Travel Requirements

This role requires travel to multiple counties and serves the far-western Northwestern Ohio region including Defiance, Van Wert, and Paulding Counties. Travel up to a 1-hour radius is required. Willingness to support occasional travel up to 2 hours is highly desired. Additional hours are available with expanded travel radius if desired, though not required. Round-trip, door-to-door mileage reimbursement is provided. A current driver's license and reliable transportation are required.

Required Qualifications

  • Active RN license in Ohio with ability to practice in the state
  • Minimum one year of paid experience at or with a home health agency or in-home care provider (or equivalent)

  • Experience in supervisory roles, wound care, lab draws, and OASIS assessments

Note: RNs with nights-only and weekends-only availability cannot be considered at this time.
