Remote Nurse Practitioner, Telehealth, Part Time, Weekends

ChenMed is seeking a Remote Nurse Practitioner to provide virtual primary care through telehealth. In this part-time role, you will conduct diagnostic patient assessments via video conference and telephone, serving as the dispositional authority for after-hours and weekend clinical calls. You will assess, diagnose, and treat acute and non-acute clinical problems while maintaining comprehensive documentation of patients' physical and psychosocial health status.

Clinical Setting

This is a fully remote telehealth position supporting primary care patients. You will work collaboratively with patients' primary care physicians and other medical providers to ensure compliance with clinical guidelines and optimal patient outcomes.

Schedule

This is a part-time position with the following weekly schedule:

• Friday: 3:30 PM - 4:30 PM
• Saturday: 12:30 PM - 12:00 AM
• Sunday: 12:30 PM - 12:00 AM

Responsibilities

• Assess acute and non-acute clinical problems through virtual consultation, performing and documenting physical evaluations and patient histories
• Analyze trends in patient conditions and develop, document, and implement patient management plans based on clinical findings
• Develop plans of care that may include health education, physician referrals, case management referrals, and patient/family counseling
• Plan patient care based on knowledge of the patient population and applicable protocols, considering cultural background, level of understanding, personality, and support systems
• Serve as a patient advocate
• Collect comprehensive and focused data relating to health needs of patients and families
• Confer with patients' primary care physicians and other medical providers to report health data and ensure guideline compliance
• Collaborate with on-call physicians to support expected clinical outcomes and implement appropriate protocols
• Evaluate progress toward expected outcomes
• Work with key contributors to enhance the quality of telehealth practices through data demonstrating program effectiveness
• Communicate using a variety of formats, tools, and technologies to build professional relationships and deliver care
• Utilize appropriate resources to plan and provide services that are safe, effective, and financially responsible
• Provide excellent customer service and professionalism to all internal and external customers
• Participate in clinical rounds, conferences, risk and quality management programs, and relevant meetings as needed
• Adhere to departmental goals, standards of performance, regulatory compliance, quality patient care compliance, policies, and procedures
• Practice in accordance with a written or electronic practice agreement
• Participate in formulation of telehealth policies, procedures, and protocols
• Initiate and participate in quality improvement activities
• Participate with committees to support growth
• Provide feedback regarding the practice of others to improve patient care
• Coordinate services with other programs
• Perform other duties as assigned and modified at manager's discretion

Required Qualifications

• Bachelor's degree in Nursing (BSN) and graduate of a school of nursing for Advanced Practice Nursing with certification in area of specialty
• Master's degree in Nursing
• Board certification by AANP or ANCC
• Basic Life Support (BLS) certification from the American Heart Association or American Red Cross upon hire
• Multi-state licensure to include Florida and at least 3 licenses in the following states: Virginia, Georgia, Michigan, Missouri, Ohio, Pennsylvania, Tennessee, Texas, Illinois, or Louisiana
• Minimum of 4 years of acute/primary care clinical work experience as a Nurse Practitioner
• Current DEA number from the DEA for schedule II-V controlled substances may be required based on state of practice
• Expert-level business acuity
• Expert knowledge and understanding of general/core job-related functions, practices, processes, procedures, techniques, and methods
• Knowledge and understanding of medical practices to function independently as a certified practitioner and in collaboration with licensed physicians, specialists, and other medical providers
• Demonstrated record of consistently achieving clinical performance metrics
• Technical capability to conduct telemedicine visits in accordance with state and federal regulations
• Ability to demonstrate excellent clinical judgment
• Ability to problem solve
• Ability to prioritize and work under pressure
• Ability to provide constructive feedback
• Ability to communicate and collaborate with physicians, patients, and other team members in a professional manner
• Ability to operate effectively with a multidisciplinary team
• Proficient skill in Microsoft Office Suite products including Excel, Word, PowerPoint, and Outlook; competent in keyboarding and other systems required for the position
• Spoken and written fluency in English
• Ability and willingness to obtain independent/autonomous practice as an NP in applicable states
• Ability to acquire and maintain knowledge and skills to maintain expertise in area of practice

Preferred Qualifications

• Minimum of 2 years of telehealth work experience
• Bilingual fluency in Spanish or Creole

Travel Requirements

• Ability and willingness to travel to attend meetings and trainings up to 10% of the time
• If supporting patients in Georgia, required to travel to Georgia on a quarterly basis (or as mandated by law) for onsite observation and medical record review by the respective delegating physician
• If supporting patients in Tennessee, required to travel to Tennessee on a bi-annual basis (or as mandated by law) for onsite observation and medical record review by the respective delegating physician
• Required to adhere to any other state laws which may require travel for onsite observation by the respective delegating physician
• Minimum requirement to work four holidays in the calendar year

Compensation

Salary range: $111,140 - $158,771 annually. Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors. This position may also be eligible for bonuses or commissions.
