Regsitered Nurse Home Health Hiring Now

Granite Peak Home Health is hiring Registered Nurses to provide skilled nursing services to patients in their homes. In this role, you will conduct initial patient evaluations, develop and implement plans of care, perform specialized nursing procedures, and coordinate with an interdisciplinary team of therapists and social workers. You will work independently, exercise clinical judgment, and maintain detailed clinical documentation including OASIS assessments.

Responsibilities

• Conduct initial patient evaluations and regularly reassess nursing needs
• Develop, implement, and revise individualized plans of care
• Provide specialized nursing services requiring advanced clinical skills
• Initiate preventive and rehabilitative nursing procedures
• Prepare clinical notes, progress notes, and care conference summaries for each patient visit
• Coordinate services across the care team
• Communicate changes in patient condition and needs to appropriate personnel
• Counsel patients and families on meeting nursing and related healthcare needs
• Process physician orders and notify physicians of patient needs and condition changes
• Complete certification, recertification, and discharge documentation
• Refer patients to physical therapists, speech language pathologists, occupational therapists, and medical social workers as appropriate
• Supervise and teach other nursing personnel
• Conduct patient care conferences
• Participate in peer review, quality assurance, and performance improvement activities
• Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change documentation within agency timeframes
• Provide total patient care as needed
• Take on-call duty nights, weekends, and holidays as assigned
• Participate in in-service programs

Required Qualifications

• Graduate of an approved school of professional nursing
• Current Montana RN license
• Two years of nursing experience
• BLS/CPR certificate
• Ability to exercise initiative and independent judgment
• Familiarity with healthcare regulations, HIPAA compliance, and patient privacy requirements
• Knowledge of appropriate software including Microsoft Word, Excel, Outlook, and Wellsky (EHR)

Preferred Qualifications

• One year of home health nursing experience
• Clean driver's license with clean driving record and vehicle insurance

Schedule

• Monday to Friday
• Weekends as needed
• On-call duty nights, weekends, and holidays as assigned
