Join a skilled nursing team at Puyallup Post Acute, a facility blending short-term rehabilitation and long-term care. In this role, you will deliver hands-on nursing care, lead a team of LPNs and CNAs, and help residents heal and thrive in a supportive, resident-centered environment. You will have real clinical ownership in resident care decisions and work collaboratively with an interdisciplinary team.
Clinical Setting
This is a fast-paced skilled nursing facility serving both short-term rehabilitation and long-term care residents. You will work in an environment requiring standing, walking, lifting up to 50 lbs, and quick response in emergencies.
Responsibilities
- Perform comprehensive nursing assessments at admission and throughout each resident's stay
- Build and update individualized care plans alongside the interdisciplinary team
- Administer medications, IV therapy, tube feedings, wound care, and other prescribed treatments
- Monitor residents closely and respond quickly to changes in condition
- Supervise and delegate to LPNs and CNAs
- Keep documentation accurate and timely in the electronic medical record
- Communicate with physicians, nurse practitioners, and families as a trusted care partner
- Participate in care conferences and discharge planning
- Uphold CMS guidelines and infection control standards
- Support quality assurance and performance improvement efforts
- Educate and reassure residents and their families
Required Qualifications
- Active, unrestricted RN license in Washington
- Graduate of an accredited nursing program (ADN or BSN)
- Current CPR/BLS certification
- Strong assessment, critical thinking, and communication skills
- Solid grasp of geriatric nursing and rehabilitative care principles
Preferred Qualifications
- One year of nursing experience
- Long-term care or skilled nursing facility experience
