The opportunity

Job Description

Pathways Healthcare, a physician- and nurse practitioner-led home health and hospice organization, is seeking a Registered Nurse or Licensed Practical Nurse for home health visits in the Arlington, Belmont, Lexington, Waltham, and Watertown areas. This is a per diem position based out of our Stoneham, MA office. We are committed to patient-centered care and have invested in AI-assisted documentation technology that reduces paperwork by 5 to 10 hours per week, allowing clinicians to focus on clinical judgment and patient care.

Responsibilities

  • Initiate appropriate preventive and rehabilitative nursing procedures
  • Administer medications and treatments as prescribed by physicians
  • Conduct initial assessments of patients and families to determine home care needs
  • Perform thorough physical assessments and gather comprehensive medical histories
  • Develop and revise individualized plans of care based on patient status and evolving needs
  • Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust plans of care
  • Responsively address inquiries and concerns from patients, caregivers, and medical teams
  • Provide counseling and support to patients and caregivers before, during, and after treatment
  • Supervise home health aides as required
  • Identify discharge planning needs and proactively implement them as part of care plan development

Required Qualifications

  • Active RN or LPN license in Massachusetts
  • Nursing degree from an accredited institution
  • Valid driver's license and reliable, insured vehicle
  • Current BLS CPR certification
  • Working knowledge of healthcare laws and regulations
  • Excellent bedside manner and exceptional communication skills

Preferred Qualifications

  • Two or more years of nursing experience with a focus on geriatrics

Schedule

  • Flexible schedule with no nights and optional weekends
  • Training, meetings, and other hourly activities paid at $35 per hour
