Registered Nurse - Weekend Start of Care Home Health

BAYADA Home Health Care is seeking an experienced Registered Nurse for per diem weekend Start of Care home health visits in Montgomery County, Maryland. As a home care RN, you will be an integral member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care while remaining at home.

Clinical Setting

You will make home visits to clients in designated geographic territories, working independently to manage time effectively and travel to cases as assigned.

Responsibilities

• Follow designated care plans in accordance with patient needs
• Make home visits to clients in assigned geographic territories
• Administer medications, perform wound care, treatments, and procedures
• Monitor clients' conditions and report changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Provide case management and coordination
• Accurately document observations, interventions, and evaluations using electronic medical records on a state-of-the-art touch pad tablet

Required Qualifications

• Current active Registered Nurse license in Maryland
• Minimum one year of recent, verifiable clinical (medical/surgical) experience
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
• Ability to work independently and manage time effectively
• Strong interpersonal skills
• Solid computer skills
• Ability to travel to cases as assigned

Preferred Qualifications

• Prior home care experience
• Prior experience with electronic medical records (EMR)
• Home Health Experience and Medicare OASIS experience
