Advocate Health is seeking a Registered Nurse for a weekend home health nursing position serving the Lincolnton region. This full-time role provides skilled nursing care to patients in their homes, conducting comprehensive assessments, developing and updating individualized care plans, and collaborating with physicians and interdisciplinary teams to ensure continuity of care.
Clinical Setting
This position is part of the Home Health Nursing department, providing direct patient care in patients' homes throughout the Lincolnton region. You will work as part of an interdisciplinary team focused on delivering quality home-based nursing services.
Responsibilities
- Perform all scheduled home visits and collaborate with interdisciplinary team members as necessary
- Conduct thorough physical, psychosocial, and environmental assessments of assigned patients
- Develop, update, and revise individualized care plans based on assessment findings and patient response to treatment
- Communicate with physicians to confirm and update patients' treatment plans, and relay changes to the RN Case Manager
- Provide ongoing assessment of patient response to treatments, medications, and nursing interventions
- Partner with the RN Case Manager on discharge planning and coordinate community resources to ensure continuity of care
- Accurately document and update patient records according to agency, state, and federal guidelines
- Communicate problems, plans, actions, and goals clearly to physicians and the RN Case Manager
- Follow published policies and procedures and monitor updates regularly
- Perform patient positioning and transporting duties using proper technique, including lifting and pushing/pulling
- Demonstrate age-appropriate knowledge and skills for the patients served, including understanding of growth and development across the lifespan
Schedule
- Friday, Saturday, and Sunday
- 12-hour shifts
- On-call duties included
- 36 hours per week
- Weekend program premium pay
Required Qualifications
- Active Registered Nurse license issued by the state in which you practice
- Basic Life Support (BLS) for Healthcare Providers certification from the American Heart Association (AHA) upon hire (unless determined not required by department leadership)
- Valid driver's license issued by the Division of Motor Vehicles
- Completion of an accredited or approved nursing program
- Typically 1 year of nursing experience
- Proficiency in clinical skills with ability to work under direction and make sound judgments
- Excellent communication and interpersonal skills
- Ability to develop rapport and maintain positive, professional relationships with patients, staff, and physicians
- Demonstrated ability to make critical, independent decisions
- Strong organization, prioritization, and problem-solving skills
- Ability to multitask with frequent interruptions
Physical Requirements
- Ability to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday
- Lift up to 50 lbs. from floor to waist
- Lift up to 20 lbs. over the head
- Carry up to 40 lbs. a reasonable distance
- Push/pull with 30 lbs. of force
- Perform a sliding transfer of 150 lbs. with a second person present
- Functional speech, vision, hearing, and touch with ability to use fine hand manipulation skills for procedures
- Potential exposure to chemical and hazardous waste, blood and body fluids, and communicable disease; protective clothing and equipment must be worn as necessary
- Ability to operate all equipment necessary to perform the job
Compensation and Bonuses
- Pay range: $35.50 - $53.25 per hour
- Sign-on bonus up to $3,000.00 for qualified applicants
- Internal referral bonus of $500.00 for current teammates
- Premium pay for weekend shifts and on-call duties
