This position is a weekend Registered Nurse role in home health, providing direct patient care in patients' homes. You will conduct comprehensive physical, psychosocial, and environmental assessments, develop and update individualized care plans, and collaborate with physicians and the interdisciplinary team to ensure continuity of care.
Responsibilities
- Perform all scheduled home visits and collaborate with the interdisciplinary team as necessary
- Conduct thorough physical, psychosocial, and environmental assessments of assigned patients
- Develop, update, and revise care plans based on assessment findings and patient response to treatment
- Communicate with physicians to confirm and update treatment plans, and relay changes to the RN Case Manager
- Provide ongoing assessment of patient response to treatments, medications, and teaching
- Partner with the RN Case Manager on discharge planning and coordinate community resources to ensure continuity of care
- Accurately document and update patient records according to agency, state, and federal guidelines
- Communicate problems, plans, actions, and goals to physicians and the RN Case Manager
- Follow published policies and procedures and monitor updates
- Perform patient positioning and transporting duties, including lifting and pushing/pulling using proper technique
- Demonstrate age-appropriate knowledge and skills for the patient population served
Required Qualifications
- Active Registered Nurse license issued by the state in which you practice
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) upon hire, unless the department leader determines it is not required
- Valid driver's license issued by the Division of Motor Vehicles
- Completion of an accredited or approved nursing program
- Typically 1 year of nursing experience
- Proficiency in clinical skills with the ability to work under direction and make sound judgments
- Excellent communication and interpersonal skills
- Ability to develop rapport and maintain positive, professional relationships with patients, staff, and physicians
- Demonstrated ability to make critical, independent decisions
- Strong organization, prioritization, and problem-solving skills
- Ability to multitask with frequent interruptions
Physical Requirements
- Ability to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday
- Lift up to 50 lbs. from floor to waist
- Lift up to 20 lbs. over the head
- Carry up to 40 lbs. a reasonable distance
- Push/pull with 30 lbs. of force
- Perform a sliding transfer of 150 lbs. with a second person present
- Functional speech, vision, hearing, and touch with ability to use fine hand manipulation skills for procedures
- Ability to operate all equipment necessary to perform the job
Working Conditions
- May be exposed to chemical and hazardous waste, blood and body fluids, and communicable disease; protective clothing and equipment must be worn as necessary
- Weekend schedule required
- Valid driver's license and reliable transportation required for home visits
