Registered Nurse - VNA PRN

Join our home health team as a Registered Nurse Case Manager providing compassionate care to patients in their homes. In this full-time role, you will oversee the implementation and ongoing assessment of patients' plans of care, manage a team of home health aides, LPNs, RNs, and other caregivers, and ensure continuity of care that promotes preventative health and reduces hospital readmissions.

Responsibilities

• Oversee the implementation and ongoing assessment of the patient's plan of care through management of home health aides, LPNs, RNs, and other caregivers
• Ensure accurate evaluation and develop and implement comprehensive care plans that address patient needs
• Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve
• Work to decrease readmissions by promoting preventative care and ensuring continuity of care

Required Qualifications

• Current license to practice as a Registered Nurse (RN)
• Current CPR certification
• Valid state-issued driver's license
• Automobile insurance and reliable transportation; willingness to travel
• One year of professional experience practicing as a Registered Nurse in a home health or similar setting

Education

• Associate's Degree in Nursing (ADN) or Associate of Applied Science in Nursing (AAS) required
• Bachelor of Science in Nursing (BSN) preferred

Schedule

• Full-time position
• Day shift, Monday to Friday
