Registered Nurse

The W Group Assisted Living Facility is seeking a Full Time Registered Nurse to provide clinical leadership and care coordination in an adult care facility setting. In this role, you will conduct patient and family assessments, coordinate services provided by other healthcare providers, and ensure compliance with state regulations.

Clinical Setting

You will work in an assisted living facility serving senior and adult care populations in New York State.

Responsibilities

• Conduct comprehensive patient and family assessments
• Coordinate services delivered by other healthcare providers
• Ensure adherence to state regulations and facility policies
• Communicate effectively with clients, residents, and families from diverse backgrounds
• Support residents who may have physical, sensory, and mental impairments

Required Qualifications

• Active New York State RN license
• Knowledge and understanding of home care policies and procedures
• Outstanding interpersonal skills with ability to communicate effectively with clients, residents, and families from diverse backgrounds who may have physical, sensory, and mental impairments

Preferred Qualifications

• Experience working in an adult care facility or assisted living program
