Registered Nurse

Samash Holistic Home Care LLC is seeking a Registered Nurse to deliver skilled nursing care to patients in their homes throughout the Medford, NY area. This role combines clinical expertise with a holistic, patient-centered approach to support whole-person wellness and individualized care.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans
• Administer medications, treatments, and skilled nursing procedures in home care settings
• Monitor and document patient health status, progress, and changes in condition
• Coordinate care with physicians, therapists, and other healthcare team members
• Educate patients and family members on health management, medications, and holistic wellness practices
• Provide wound care, IV therapy, and other specialized nursing interventions as needed
• Ensure compliance with all state, federal, and agency regulations and standards of care

Required Qualifications

• Active and unrestricted Registered Nurse (RN) license in New York
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Reliable transportation and valid driver's license for home visits in the Medford, NY area
• CPR/BLS certification
• Ability to work independently in a home care environment

Preferred Qualifications

• Previous home care, community health, or clinical nursing experience
