This position is part of the Roving Community Health Initiative, providing nursing care to adults with complex medical and behavioral health needs in community-based and street medicine settings across San Francisco. You will work independently in the field while collaborating with an interdisciplinary team to deliver safe, therapeutic care to individuals experiencing homelessness and substance use disorders.
Responsibilities
- Utilize the nursing process for assessment, planning, implementation, and evaluation
- Provide direct nursing care for patients with complex medical and behavioral health needs
- Work as a member of the interdisciplinary and nursing teams
- Carry out established nursing procedures and related duties as required
- Perform nursing care with assistance and supervision as needed
- Assist with management of patient care administered by other nursing team members
- Maintain knowledge and abilities essential to the role
- Carry out legal responsibilities within the scope of the Nurse Practice Act
Clinical Setting
This role requires flexibility to orient and work at all UCSF Medical Center locations and off-site locations across San Francisco. The position involves local travel and driving to community-based and street medicine settings.
Required Qualifications
- Two years of recent RN experience (within the past five years) caring for adults with complex medical and/or behavioral health needs
- Experience working with substance use disorders and/or mental health conditions in community-based or street medicine settings
- Ability to work independently in the field while collaborating effectively within a multidisciplinary team
- Valid California Driver's License; position requires local travel and driving
Preferred Qualifications
- Knowledge of harm reduction principles and trauma-informed care for individuals experiencing homelessness and substance use disorders
- Skill in crisis assessment, de-escalation, and stabilization of patients with acute behavioral health needs
- Knowledge of community resources and care navigation, including linkage to primary care, behavioral health, and social services
- Strong interpersonal and communication skills with the ability to build trust across diverse and marginalized populations
Licenses and Certifications
- Current licensure as a Registered Nurse in the State of California
- Current healthcare provider CPR certification
- Valid California Driver's License
