Registered Nurse RN - Universal Home Health Case Manager

Otterbein Universal Home Health is seeking a Registered Nurse (RN) Case Manager to provide skilled professional nursing care to patients, families, and caregivers in the home health setting. This role manages a primary caseload while ensuring compliance with Medicare Conditions of Participation, Ohio Home Health licensure laws, and agency policies.

Responsibilities

• Manage a primary caseload of home health patients
• Provide skilled professional nursing care as prescribed by the physician
• Ensure compliance with Home Health Medicare Conditions of Participation and Ohio Home Health licensure requirements
• Work with patients, families, and caregivers to deliver person-centered care

Schedule

• Full-time position
• 8:00 AM to 5:00 PM
• PRN (as needed) availability

Required Qualifications

• Valid Ohio Registered Nurse (RN) license
• Minimum of one year of direct patient care experience in home care
• Graduate of an approved nursing program

• Bachelor's degree in nursing
