Registered Nurse RN PRN Home Health

Elara Caring is seeking a Registered Nurse for PRN (as-needed) home health positions. In this role, you will provide direct nursing care to patients in their homes, conducting assessments, implementing care plans, and monitoring patient conditions while maintaining the highest standards of safety and quality.

Responsibilities

• Assess patients to determine their care needs and complete initial and ongoing patient assessments
• Provide nursing care according to the plan of care and physician orders
• Monitor patient conditions and report changes to the care team
• Educate patients and families on their care and safety
• Take appropriate action when patient conditions change
• Help ensure quality, safe, and continuous patient care
• Protect patient and staff privacy in accordance with HIPAA

Required Qualifications

• Current State license as a Registered Nurse
• Graduate of an accredited Certificate, Diploma, Associates, or Baccalaureate School of Nursing
• One or more years of experience as a nurse in a clinical care setting
• Valid driver's license with an acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
