The opportunity

Job Description

Vertus HealthCare is seeking experienced Registered Nurses to provide skilled nursing care to patients in their homes throughout Palm Beach County. This is a per diem position offering flexible scheduling for RNs who want to work on an as-needed basis.

Responsibilities

  • Provide skilled nursing visits based on the patient's plan of care
  • Complete OASIS, SOC, ROC, Recertification, and Discharge visits as assigned
  • Assess patient needs and coordinate care with the clinical team
  • Complete timely and accurate clinical documentation
  • Communicate with physicians, patients, families, and the healthcare team

Required Qualifications

  • Current Florida RN license
  • Reliable transportation

Preferred Qualifications

  • Home health nursing experience
  • OASIS experience
  • Strong clinical assessment and documentation skills
