Registered Nurse (RN) Patient Care Navigator - Structural Heart

Inova Health Center 8081 – Fairfax East is seeking a Registered Nurse (RN) Patient Care Navigator - Structural Heart to join the team. This full-time position focuses on coordinating comprehensive care for structural heart patients, serving as a liaison between patients and the healthcare system to ensure access to appropriate services, treatment options, and support resources.

This role is based at Inova Health Center 8081 – Fairfax East, a nationally recognized healthcare leader in safety, quality, and patient experience.

Schedule

• Full-time position
• Monday – Friday
• No holidays
• No weekends

Responsibilities

• Coordinate patients' plans of care, including appointment scheduling, confirming attendance, arranging transportation, assisting with referrals, and answering patient questions
• Serve as a liaison, advocate, and primary point of contact for patients seeking screening, diagnostic treatment, clinical research participation, support services, and follow-up or aftercare planning
• Refer cases to appropriate resources and anticipate patient needs while advocating for patient rights and appropriate service utilization
• Conduct comprehensive assessments of patients' health and psychosocial needs and develop case management plans in collaboration with patients and their support systems
• Educate patients, families, caregivers, and healthcare team members regarding treatment options, community resources, insurance benefits, self-care management, and other relevant information to support informed decision-making
• Collaborate with multidisciplinary teams to address patient questions and concerns about care processes and treatment options
• Develop and maintain reliable systems to document, track, and monitor patient data
• Perform other duties as assigned

Required Qualifications

• Bachelor's Degree in Nursing
• 3 years of nursing experience
• Active license as a Registered Nurse in the Commonwealth of Virginia
• Basic Life Support (BLS) certification from the American Heart Association

Preferred Qualifications

• Cardiac nursing experience
• Familiarity with cardiac medications
