Registered Nurse (RN) - Part-Time

CareAparent is seeking a part-time Registered Nurse to provide in-home nursing care to seniors in the Twin Cities or Saint Cloud metro areas. In this role, you will conduct client assessments and re-evaluations, assist in developing care plans, perform medication set-ups, and supervise home health aides and certified nursing assistants. You will be responsible for identifying and implementing individualized home health care plans, educating patients, families, caregivers, and other providers on safe care delivery, and ensuring compliance with home health episodes and authorizations. CareAparent's Case Management model provides timely training and support to help you deliver excellent care.

Clinical Setting

• In-home care for seniors throughout the Twin Cities metro area or Saint Cloud metro area
• Travel between multiple client homes

Responsibilities

• Conduct in-home client assessments and re-evaluations
• Assist in preparing individualized care plans
• Perform medication set-ups
• Supervise home health aides and certified nursing assistants
• Identify and create home health care plans for admission under home health episodes and authorizations
• Educate patients, families, caregivers, and other providers on safe care delivery
• Ensure compliance with care plan implementation

Required Qualifications

• Current Minnesota RN license
• Previous experience with seniors and client assessments
• Excellent customer relations and interpersonal skills
• Excellent verbal and written communication skills
• Valid driver's license and insured personal vehicle
• Willingness to travel to and from client homes

Compensation and Schedule

• Pay range: $39 to $45 per hour
• Part-time position
