Preferred Care Home Health is seeking a Registered Nurse (RN) to join the One Heart Program, a specialized cardiac and cardiopulmonary home health service. In this role, you will provide skilled nursing care to cardiac and cardiopulmonary patients in their homes, working independently while collaborating with physicians, an APRN, therapists, and the interdisciplinary care team. This position is ideal for an RN who enjoys patient education, care coordination, and making a direct impact on patient outcomes and hospital readmission reduction.
Responsibilities
- Provide skilled nursing care to cardiac and cardiopulmonary patients
- Complete comprehensive assessments and develop individualized care plans
- Monitor patient progress and communicate changes in condition to the care team
- Collaborate with physicians, APRN, therapists, and interdisciplinary team members
- Educate patients and caregivers on medications, disease management, safety, and recovery
- Assist with Remote Patient Monitoring (RPM) setup and patient education
- Administer skilled nursing treatments, including IV Lasix for eligible CHF patients
- Maintain accurate and timely clinical documentation
Required Qualifications
- Active Florida RN license
- Graduate of an accredited nursing program
- Minimum 1 year of medical-surgical nursing experience
- Current CPR certification
- Valid driver's license, reliable transportation, and auto insurance
Preferred Qualifications
- Cardiac, telemetry, cardiovascular, cardiothoracic, or home health experience
- Strong assessment, communication, and care coordination skills
