Interim HealthCare, the nation's first home care company, is seeking Registered Nurses to provide home-based care to patients in the Madison, Wisconsin area and surrounding communities. This role offers autonomy, meaningful one-on-one patient care, and a flexible schedule that supports work-life balance. You will work with 1:1 nurse-to-patient ratios and have the opportunity to choose between Home Health or Hospice specialties.
Clinical Setting
You will provide care in patients' homes, working as part of an interdisciplinary clinical team. Home Health nurses care for patients with illnesses, injuries, and chronic diseases, often following discharge from hospitals, rehabilitation facilities, or nursing homes. Hospice nurses provide comfort-focused care to patients with six months or less to live, emphasizing quality of life and engaging the patient's mind, body, spirit, and family.
Responsibilities
Home Health Registered Nurses:
- Provide home-based care to patients with illnesses, injuries, and chronic diseases
- Examine patients, perform assessments, document progress, and report changes to physicians
- Oversee LPNs, CNAs, and aides providing patient care
- Educate patients on their plan of care, prescribed medications, therapy, diet, and exercise
- Coach family members on the patient's plan of care, medication, and home safety
- Ensure care goals are met and coordinate discharge from services
Hospice Registered Nurses:
- Provide home-based care to patients with six months or less to live, focused on quality of life
- Work as part of an interdisciplinary team including physicians, LPNs, CNAs, HHAs, social workers, chaplains, and volunteers
- Perform patient assessments, check vitals, evaluate breathing, and monitor pain
- Administer prescribed medications and treatments
- Ensure accessibility of medical equipment, prescribed oxygen, medications, and other supplies
- Educate families on patient prognosis, changes in care plan, medications, and hospice goals
Required Qualifications
- Graduate of an accredited nursing program
- Active RN license in Wisconsin
- Minimum of 1 year of nursing experience
- CPR certification
- Valid driver's license and auto insurance
- Knowledge of state and federal home health or hospice regulations
- Clinical proficiency in nursing specialty
- Strong communication skills
Preferred Qualifications
- Prior experience in home healthcare or hospice
Compensation and Benefits
- Salary: $80,000 - $95,000 per year
