Registered Nurse, RN Home Visits - Weekend

BAYADA Home Health Care is seeking a Registered Nurse (RN) to join our home health visit team providing skilled nursing and rehabilitative care to clients in their homes. This is a weekend position focused primarily on Start of Care (SOC) and admission visits. You will work independently in varied home environments, conducting comprehensive assessments, managing clinical situations, and providing direct patient care while maintaining detailed OASIS documentation.

Clinical Setting

You will provide home-based skilled nursing care to adult and geriatric patients across the office's entire coverage area. The role requires flexibility to travel throughout the service territory and work in diverse home environments.

Responsibilities

• Conduct head-to-toe assessments and identify subtle changes in patient condition
• Perform Start of Care (SOC) and admission evaluations
• Manage acute and post-acute care needs including CHF, COPD, wound care, IV therapy, and post-surgical care
• Triage and manage urgent clinical situations independently in the home setting
• Perform medication reconciliation, patient teaching, and safety monitoring
• Conduct comprehensive wound assessment, care, and documentation
• Complete point-of-care documentation and OASIS requirements
• Work as part of a multidisciplinary healthcare team
• Manage a dynamic visit schedule and prioritize urgent visits

Schedule

Weekend position with flexible scheduling options:

• Part Time: Commit to 4 SOC per weekend (2 Saturday + 2 Sunday) with $5,000 incentive opportunity
• Full Time: Commit to 20 productivity points per weekend (Friday-Monday) with $5,000 incentive opportunity and benefits eligibility

Required Qualifications

• Current, active RN license in good standing in Colorado
• Graduation from an accredited and approved nursing program (school transcript or diploma required)
• At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in one of the following settings: acute care, emergency department, home health, hospice, ICU, medical-surgical, sub-acute care, or rehabilitation units
• Home Health and OASIS documentation experience required
• Ability to work independently while knowing when to escalate concerns
• Comfortable making real-time clinical decisions without immediate on-site support
• Strong problem-solving skills in unpredictable home environments
• Efficient with point-of-care documentation and technology
• Strong attention to detail while maintaining productivity expectations
• Comfortable providing care in varied home environments
• Flexible and adaptable to changing patient needs and schedules

Compensation

• Competitive pay: $60-65 per point, depending on qualifications and subject to negotiation
• Weekly pay schedule
• $5,000 incentive opportunities based on schedule commitment
